Adverse Event
An adverse event is an unintended injury or complication that is caused by health care management, rather than by the patient's underlying disease and that has lead to death, disability at the time of discharge or prolonged hospital stays. (Baker, Norton et al, 2004).
Human Factors Engineering
Human factors engineering is a discipline of applying what is known about human capabilities and limitations to the design of products, systems, processes and work environments (MacGonigle and Mastrian, 2012).
Patient Safety
What is patient safety….according to the World Health Organization (WHO), “Patient safety is the absence of preventable harm to a patient during the process of health care. The discipline of patient safety is the coordinated efforts to prevent harm, caused by the process of health care itself, from occurring to patients. Over the past ten years, patient safety has been increasingly recognized as an issue of global importance, but much work remains to be one.” (World Health Organization , 2013).
Safety Culture
Safety culture is frequently defined as “the product of individual and group values, attitudes, competencies and patterns of behaviour that determine the commitment to and the style and proficiency of an organization’s health and safety programs. Organizations with a positive safety culture are characterized by communications founded on mutual trust, by shared perceptions of the importance of safety, and by confidence in the efficacy of preventative measures.”(Canadian Patient Safety Institute, 2012).
Workaround
A workaround is a plan or method to circumvent a problem without eliminating it (Merriam-Webster Dictionary, 2013).
